This agreement outlines the terms and conditions under which I consent to Cheswick Consultants Pty Ltd storing my personal information, including health information, in a cloud-based environment, in accordance with the Privacy Act 1988 and the Australian Privacy Principles (APPs).

By signing this agreement I understand and agree to the following:

1. Consent to Cloud Storage: I consent to Cheswick Consultants storing my personal information, including health information, in a cloud-based storage system.

I understand that this information will be digitised and uploaded to remote servers managed by Cheswick Consultants.

2. Purpose of Storage: I understand that my information will be stored for the purpose of Cheswick Consultants providing me with healthcare services and managing my health records. I understand that Cheswick Consultants' access to my information is limited to providing data storage and access services.

3. Data Security and Privacy: I understand that Cheswick Consultants are obligated to take reasonable steps to protect my personal information from misuse, interference, loss, unauthorised access, modification, or disclosure, in accordance with the APPs. I am aware that there are potential risks associated with cloud storage, including the risk of unauthorised access or data breaches. I understand that my information will only be disclosed in accordance with the law and my prior consent.

4. Access and Control: I understand that I have the right to access and request corrections to my personal information held by Cheswick Consultants. I understand that Cheswick Consultants maintains effective control over my personal information stored online.

5. Breach Notification: I understand that I will be notified by Cheswick Consultants in the event of a data breach involving my personal information, in accordance with the Notifiable Data Breaches scheme.

6. Agreement Review: I understand that this agreement may be reviewed periodically to ensure its continued compliance with applicable laws and regulations.
